Myth Busting: Are nicotine pouches healthier than smoking?

Nicotine pouches. You've seen the small round tins, but what's in them? A pouch is a teabag-like sachet that sits between your lip and gum and releases nicotine into your bloodstream. It holds nicotine powder, flavouring and filler, but no tobacco leaf.

Today it's legal to sell one to a child in the UK. Pouches sit outside the rules that cover tobacco and vapes, so anyone of any age can buy one at any strength. That changes on 29 October 2026, when an age limit of 18 comes in.

In this article of Emerald's myth busting series, we look at the claim that nicotine pouches are healthier than smoking.

What the claim says

The tin says tobacco-free, and it's easy to read that as risk-free. The claim grows from there. Pouches are fine. Some people treat them as a focus aid.

You'll have met this one in the gym or halfway through a football match. A pouch is odourless and leaves nothing behind, so it goes where a cigarette can't. And the advertising ban doesn't arrive until June 2027.

Use has climbed tenfold in five years, from 1 in 1,000 British adults in 2020 to about 1 in 100 in 2025, or roughly 522,000 people. Among men aged 16 to 24 it's 1 in 13. About 1 in 30 children aged 11 to 17 have tried one.

What the claim gets right

Almost everything that makes smoking deadly comes from setting tobacco alight. Burning produces tar, carbon monoxide and carcinogens. A pouch burns nothing, so none of that reaches you. Chemical analyses find far fewer harmful substances in pouches than in cigarette smoke.

Action on Smoking and Health says pouches are likely to be less harmful than smoking. In June 2026, United States regulators went further and allowed certain pouches to be sold with a claim that using them instead of cigarettes lowers your risk of mouth cancer, heart disease, lung cancer and stroke.

Where the claim falls down

Safer than smoking is a very low bar. Smoking is the deadliest consumer product ever sold, and clearing that bar tells you little about whether something is good for you. It only counts if cigarettes are what you'd otherwise be using.

For most users they aren't. About 7 in 10 British pouch users also smoke or vape, so pouches are going in alongside cigarettes rather than instead of them. Topping up your nicotine during the parts of the day you can't smoke does nothing about the cigarettes you still have. About 1 in 6 users never regularly smoked at all, and for them the honest comparison is pouches against nothing



Nicotine has effects of its own. It raises your heart rate and your blood pressure. The British Heart Foundation discourages every nicotine product for anyone who already has a heart condition, and burning has nothing to do with that. What we don't have yet is long-term data on pouch users specifically.

Your gums notice too. A pouch sits in the same spot for half an hour or more at a time, and the likely cost is soreness, ulcers and gum recession where it sits. The research here is thin. And nicotine itself isn't linked to mouth cancer, so keep this one in proportion.

The dose is unregulated. Licensed nicotine gum comes in 2mg and 4mg strengths, the doses it was tested at. Pouches have no legal limit. Nobody can see or smell one either, so dependence is easy to build and easy to miss.

Emerald's stance

Emerald's position is simple. If you smoke, switching completely to pouches is very likely better than carrying on, but pouches aren't a stop smoking treatment and they aren't something to take up.

The evidence for quitting is thin. The 2025 Cochrane review found four studies covering 284 people between them, none running longer than eight weeks. In the one trial that compared the two directly, pouch users may have been slightly less likely to quit than those using an e-cigarette, though the study was too small to be sure. The trial figures are averages, not promises.

So pouches aren't licensed as stop smoking medicines here, and NICE doesn't recommend them. If you want to stop smoking, licensed nicotine replacement, varenicline or a regulated vape through NHS stop smoking services all rest on far stronger evidence.

Conclusion

Pouches spare you the tar, the carbon monoxide and almost all of the carcinogens. Nobody has shown that what's left is good for you.

Verdict

Nicotine pouches are healthier than smoking. Unsupported. There's no long-term data on pouch users, and no trial has shown they help you quit.

If you're using pouches to get off cigarettes, speak to your GP about options with better evidence behind them.
